In an effort to more efficiently disseminate information regarding evidence-informed skin and wound care throughout the South West Local Health Integration Network (LHIN), the South West Regional Wound Care Program (SWRWCP) asked all hospitals, long-term care homes, community nursing service provider agencies and primary care organizations to identify staff that could be designated as 'Wound Care Champions'.  The following PDF provides a formal definition of 'Wound Care Champion', and a description of their roles.

The following PDF files contain the names and contact information for 'Wound Care Champions' in the South West LHIN.  The goal of posting this information is to allow 'Wound Care Champions' the opportunity to connect with each other through e-mail, and to allow health care providers and health care consumers the opportunity to learn who their organization's 'Wound Care Champions' are and how to contact them to discuss issues pertaining to skin and wound care within their respective organizations.  Although efforts have been made to ensure the accuracy of these lists, it is ultimately up to 'Wound Care Champions' to keep the Program aware of any changes.  To report a change in your facilities designated 'Wound Care Champions', please e-mail crystal.mccallum@sw.ccac-ont.ca.
